Responsibilities
Job Summary
Under the direction of the Manager of Case Management and in collaboration with the hospital leadership team, has responsibility for daily operational management of the Care Management teams, including orientation and ongoing staff development. Is responsible for the planning, directing and team oversight of Care Management implementation at The University of Kansas Hospital. The Supervisor provides direction to the Integrated Nurse Case Managers and Social Work Case Managers.
Responsibilities and Essential Job Functions
- Identifies, monitors and reports opportunities for quality and performance improvement
- Assists with hiring and performance evaluation of the staff.
- Creates and maintains a supportive work environment for the staff.
- Makes recommendations and provides opportunities for growth and development of personnel.
- Promotes a level of professionalism within the team, both in culture and appearance. Motivates academic pursuit and continuing education on the part of the staff by establishing and meeting service-based certification goals.
- Strengthens collaborative proactive by respecting roles and professional boundaries of others, empowering individuals and teams, and offering appropriate support.
- The primary role of the case managers is to collaborate, communicate and facilitate coordination of services during hospitalization and post-hospitalization as established by the healthcare team.
- The primary role of the social workers is the for the psychosocial assessments of patients, for post hospital discharge needs including home care, nursing home placement, durable medical equipment, financial assistance, counseling, and other community resources.
- Responsible for supervising approximately 15 Nurse Case Managers and/or Social Workers.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Bachelor Degree Nursing OR
- Masters Social Work
- 5 or more years of clinical nursing or general health care experience
- 2 or more years of case management/care coordination experience
Preferred Education and Experience
- Master Degree Nursing or related health field
Required Licensure and Certification
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing in the State of Kansas OR
- Licensed Masters Social Worker(LMSW) - State Board of Behavioral Sciences in the State of Kansas
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) if RN
Preferred Licensure and Certification
- Case management certification
