Responsibilities
Job Summary
The Home Health/Hospice Wound/Ostomy Nurse utilizes the nursing process to coordinate the interdisciplinary approach to patient care for patients presenting with wounds or ostomy needs. He/she promotes continuity through communication of the patient's plan, condition, treatment, and progress toward outcomes. This individual participates in and demonstrates effective education and support for patients and their families. He/she practices within the boundaries of the Kansas Nurse Practice Act, Wound/Ostomy specialty standards, and organizational/departmental policies and procedures.
Responsibilities:
- Consistently meets work schedule.
- All other duties as assigned.
- Demonstrates critical thinking utilizing all aspects of the nursing process in all aspects of patient care (Assessment, Diagnosis, Plan, Intervention, Evaluation) in wound and ostomy issues.
- Provides appropriate patient/family teaching for wound and ostomy issues using resources available.
- Coordinates patient care though effective and professional communications with physicians, nursing and ancillary staff. Assures proper follow-up care is arranged post discharge when consulted.
- Demonstrates leadership amongst peers; acts as preceptor/mentor for teammates.
- Provides patient assessment, documentation and teaching in areas of professional nursing wound care, ostomy care and continency issues.Develops plans of care for: wounds and / or skin breakdown, ostomy care, and other skin related issues,in consultation with physicians, RN's, and therapists.
- Provides educational opportunities for nursing staff as needed. Assists with establishment and professional resource for skin and wound team, and all hospital staff.Develops and provides education for floor RN's to promote daily patient care as it relates to wound care assessment and management.
- Communicates with peers, providers, supervisory personnel, other disciplines, and internal/external referral sources regarding treatment plan, products and patient needs.
- Consults and accomplishes regular follow-up on all patients with skin breakdown, complex wounds and ostomy care issues, per policy.Assesses patients and coordinates physician consults and assures proper orders are submitted.
- Assists in establishing and maintaining policies and procedures pertinent to specialty area, based on evidenced-based research on wound, ostomy and continence issues.
Required:
- 4 Year / Bachelors Degree
- 3 - 5 Years Medical Surgical Nursing and/or Community-based or Long-Term care nursing
- Healthcare Provider CPR – (American Heart Association or American Red Cross ONLY)
- CWCN - Wound Certification
- COCN - Ostomy Certification
