The Social Worker is expected to support the mission, vision, and values of Olathe Medical Center, Inc. This position is responsible for consultation and direction of social services to meet the needs of Hospice patients. Assure that social service/counseling services are integrated and coordinated with other team members. He/She will uphold the philosophy of the Hospice Agency in meeting comfort care needs of the patient and family. Implement program components as outlined in Kansas Hospice Policies and Procedures. Leads or co-leads Grief Support Groups. Assists in the training, education, and supervision of bereavement volunteers. Assists Hospice team members and volunteers in understanding bereavement and grief process experienced by hospice families. Maintains current files of community agencies, services, and groups, which could be used as potential resources for the bereaved. Uses resources efficiently and prudently (medical office equipment, cell phone, EMR), communicates with patients and families, referral sources, insurance case managers, physicians. Actively participates in IDT weekly meetings.


Responsibilities and Essential Job Functions
  • Consistently meets work schedule.
  • Demonstrates appropriate assessment and intervention skills in meeting the psychosocial needs of Home Health and Hospice clients and families.
  • Documents all assessments, interventions, and contacts.
  • Demonstrates awareness and competence in working with clients and families within the context of disability, age, ethnicity, and culture.
  • Performs bereavement assessment and counseling per protocol to hospice families/S.O. after patient death.
  • Offers bereavement counseling to community members on an as needed basis.
  • Facilitates bereavement groups as scheduled.
  • Attends and participates in IDT meetings.
  • Communicates appropriately and effectively with clinical and office staff in meeting client needs.
  • Identifies self and other staff learning needs and participates in professional growth, i.e. teaching, education and professional associations.
  • Demonstrates the knowledge and ability to utilize community resources to meet the needs of the Hospice and Home Health patients and families.
  • Manages and directs grief support group programs and bereavement activities.
  • Provides planning and oversight for all functions of the grief support programs.
  • Adheres to CMS standards of care related to the bereavement role.
  • Demonstrates community engagement through events and educational presentations to increase awareness and access to grief support services for the entire community.
  • Maintain and provide oversight for program educational material.
  • Provides phone support: information, intakes, crisis response, and community referrals.
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
  • Master's Degree in Social Work
  • 2 or more years of experience in Social Work or a related field

Preferred Education and Experience
  • Community Based Services and /or healthcare setting

Required Licensure and Certification
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) BLS for Healthcare Providers (AHA)
