Job Summary
The Nurse Navigator – Cardiology is a multi-faceted position providing comprehensive cardiology services to patients throughout the care continuum working in the role of a registered nurse, case manager, and patient liaison. The navigator is responsible for a health system wide patient care approach monitoring and managing the patient through the spectrum of the disease process. This position is responsible for the clinical, quality, and cost effective outcomes for a caseload of patients, and for collecting, trending, and analyzing aggregated outcomes data for the cardiac patient population to be used in identifying best practices and targeting performance improvement efforts. This position requires the advanced knowledge and expertise to identify and implement improvement processes, and the ability to design, direct and implement teaching plans. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill based upon the physical, developmental, psychological, environmental, and spiritual principles. This position requires self-direction, motivation, organization, and interpretation of information relating to heart failure policies, procedures, and programs. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers. Coordinates the care for oncology patient, their family and/or caregiver across the continuum, overcoming health care barriers, providing a comprehensive assessment and facilitating timely referrals for appointments to the multidisciplinary oncology team.
Responsibilities and Essential Job Functions
- Consistently meets work schedule.
- Coordinates and participates in Service line development to reduce heart failure and myocardial infarction readmission rate and length of stay.
- Assist the medical staff in coordinating a multi-disciplinary approach to Heart failure and Myocardial infarction from conferences, office care, inpatient and outpatient care, home health, and navigation services. Communicate with physician and other health team providers and serve as liaison for cardiac patients, family members and other health team providers. Coordinate and monitor referring appointment s for follow up care and rehabilitation as needed.
- Coordinates patient multidisciplinary visits, document treatment plan summary and disseminate to patient, appropriate departments and physicians. Coordinates appointment and referral visits. Serves as the liaison and navigator for the patient between the inpatient and clinical office settings.
- Participates in committees/ conferences related to heart failure and myocardial infarction. Welcomes all phone calls from cardiology patients/family members regarding information or questions and concerns about patient care and record in patients charts.
- Manages the quality activities and data collection related to the cardiology patient.
- Develops in conjunction with support staff heart failure and myocardial infarction educational opportunities for staff, patients and families.
- Provides education related to oncology diagnosis, treatment, cancer center support services and community resources.
- Facilitates timely coordination of appointments, tests and procedures communicating any concerns to the appropriate physician(s).
- Provides a comprehensive assessment of the oncology patient, family and / or caregivers needs at pivot medical visits across the continuum, including assessing barriers to care and distress screening, refer to appropriate disciplines. (Commission on Cancer, Chapter 3)
- Develops assessment tools, guidelines, pathways and algorithms across the continuum of care.
- Educates the patient, family and/or caregiver to clinical trials, coordinate with Cancer Research Department (Commission on Cancer E9, Clinical Trial Information)
- Coordinates patient multidisciplinary (MDC) visit, document treatment plan summary and disseminate to patient, appropriate departments and physicians. Coordinates all appointments and referrals after the MDC visit.
- Participates in the multidisciplinary tumor conference and document recommendations as outlined by Commission on Cancer Standards. Monitors recommendations for appropriateness with NCCN or other national guidelines. (Commission on Cancer, Standard 1.7).
- Participates in navigation tool development for assessing patient, family and/or caregiver needs.
- Reviews national NCCN (National Comprehensive Cancer Network) or other national oncology standards with the multidisciplinary team on a yearly basis.
- Utilizes Tumor Registry for any specific data requests for program development or performance improvement initiatives.
- Participates in the completion of the cancer focused community needs assessment, present to cancer committee per Commission on Cancer Standards, and reviews on an annual basis. Navigation program needs to be modified based on the changes to the community needs assessment. (Commission on Cancer Standard 3.1)
- Participates in performance improvement activities to enhance the patient, family and / or caregiver experience across the continuum. (Commission on Cancer Standard 4.5, 4.7 and 4.8 )
- Develops in conjunction with support staff support groups and education programs for the patient, their families and/ or caregivers.
- Participates in the Patient Advisory Committee and /or Patient and Family Centered Care Committee.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Associate Degree Nursing
- 3 or more years Cardiology or Oncology Nursing experience
Required Licensure and Certification
- Licensed Registered Nurse (LRN) - Single State - State Board of Nursing upon Hire OR
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing upon Hire
- CPR/ AED/ BLS - Other BLS within 14 days
