Looking for someone interested in Part time hours ! We are seeking a Clinical Nurse Navigator (CNN) to support the delivery of a new, coordinated model of care program, called RETAINWORKS being offered by the Care Collaborative, a subsidiary of The University of Kansas Health System. RETAINWORKS is designed to help individuals return to work who lose work time because they are ill or injured on or off the job. This is a remote position enabling the CNN to work from home and commute directly to the clinics when their patients will be seen. • Primary role is to collaborate, communicate and facilitate coordination of services during enrolled patients’ recovery from work disabilities that impair their ability to work as established by the healthcare team • Partners with ambulatory care providers in communities within a defined geography in western Kansas trained in early interventions to help workers return to work • Explains the protocols to participate in the program and enroll patients who are referred by their providers • Documents and maintains regular communication with the provider and patient as interventions are completed to help ensure the patient’s continued progress • Oversees the day-to-day operations of the RETAINWORKS program in their area • Collaborates with local workforce centers who work with patients and employers to ensure training, accommodations and support services ease patients’ return to work when medically able


Responsibilities and Essential Job Functions
  • Responsible for coordinating RETAINWORKS patients’ progression through defined early interventions to align with the treatment goals and restrictions established by patients’ providers, promotes effective utilization and monitoring of healthcare resources, and guides all participants toward positive quality outcomes
  • Communicates timely, relevant and accurate information to all parties involved with patient's care: patients, families, all members of the healthcare team, and community partners
  • Monitors the patient's progression towards the desired outcome
  • Manage the care of the patient population, developing expected patient outcomes, defining appropriate resource utilization, and working with providers and workforce center partners to set patient goals, and the patient to achieve them
  • Travel within assigned geography to participate in patient encounters and reinforce patients’ successes and challenges to ensure necessary adjustments to plan
  • Provide nursing support/coordination to participating providers’ practice to optimize patient outcomes and to improve patient’s quality of life through clinical and educational programs and/or support services
  • Advocate for and develop relationships with patients via weekly telephone check-ins, competently using assessment skills to identify physical, psychosocial, financial, and environmental health care needs of the patient
  • Utilize available electronic tools, such as local EMRs, Acrobat and Microsoft Office, to efficiently document assessments, clinical finds, plans of treatment and progress toward goals
  • Observe, record and report patient's condition and reaction to drugs and treatments, escalating issues and engaging provider as appropriate
  • Practice effective time management and organization skills to ensure appropriate time is committed to documentation, completing expense/mileage reports, and patient follow-up
  • Accurately and reliably tracks and reports completed interventions/activities to enable prompt payment of providers and patients
  • Educates patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness
  • Maintains patient confidentiality
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
  • Associate Degree Nursing
  • 2 or more years of RN or LPN experiemce

Preferred Education and Experience
  • Bachelor Degree Nursing
  • Previous medical office experience

Required Licensure and Certification
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) within 14 days
  • Registered Nurse or Licensed Practical Nurse - licensed in the state in which they will be working and proof of compact licensure submitted/applied for in the state in which the employee lives in

Knowledge Requirements
  • Basic typing and word processing ability
  • Excellent analytical, critical thinking, communication (written and verbal), problem-solving and interpersonal skills
