Position Summary / Career Interest:
The Ryan White Service Coordinator, under the direction of the Ryan White Supervisor and Social Work Manager, has responsibility to provide case management services to HIV-infected individuals on an on-going basis, including assessment, enrollment into case management and assistance in acquiring needed medical/support services as established by the healthcare team and executed by the case manager.

Responsibilities and Essential Job Functions
- Accepts responsibility and accountability for achievement of optimal outcomes within their scope of practice.
- Follows policies, procedures and standards; complies with Corporate Compliance program.
- Assumes responsibility for risk and safety issues associated with the position.
- Performs job specific responsibilities and demonstrates accountability for own actions and decisions.
- Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope.
- Demonstrates knowledge of the available health and social services available to those who are HIV infected. Maintains current licensure.
- Brings ideas and concerns to supervisor, participates in department decision making.
- Collaborates as a team member to achieve organizational and department goals.
- Collaborates with the Ryan White Supervisor in meeting contract expectations from the City, including preparation of grants, budgets, annual and quarterly reports, submits financial eligibility registration forms, summary reports, monthly supervision and any other required documentation in a timely manner. Communicates timely, relevant and accurate information to all parties involved with patient's care.
- Communicates effectively and frequently with multiple customers to coordinate care. Partners with clients to develop a written case management plan for each client. Provides face-to-face contacts with case management clients in a timely manner in the clinic, hospital, clients' home and/or over the telephone.
- Educates and counsels each client about complex medication regimens and disease progression, and provides sensitive reinforcement of risk-reducing behaviors.
- Documents all HIV care coordination activities in individual client records.
- Participates as a member of a team to provide coverage for clients during absence of other case managers. Participates in partnership with the Kansas City, Missouri Health Department and other Ryan White clinics to meet the needs of this population.
- Attends meetings with the City's HIV/AIDS program, attends other required meetings, and Case Management Department meetings as requested.
- Works collaboratively with clients, families, all members of the healthcare team and community partners to develop plan for services based on identified needs. Participates in performance improvement activities.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Masters Social Work
Required Licensure and Certification
- Licensed Master Social Worker (LMSW) - State Division of Professional Registration upon Hire
