We currently have an opening for an Integrated Nursing Case Manager Lead
Accepts responsibility and accountability for achievement of optimal outcomes within their scope of practice. Follows policies, procedures and standards; complies with Corporate Compliance program. Assumes responsibility for risk and safety issues associated with the position. Takes call as required by the department expectations. Performs specific job responsibilities and demonstrates accountability for own actions and decisions.
Collaborates with leadership and departmental staff regarding changes that impact case management's role.
Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope.
Brings ideas and concerns to supervisor, participates in department decision making.
Demonstrates flexibility and teamwork among case management staff members.
o Assists peers in the event of fluxuating census.
Assists with hiring, education and performance evaluation of the staff
Identifies, monitors and reports opportunities for quality improvement to the appropriate department
o Ad-hoc member of department practice council
o Facilitates ongoing unit education and professionalism through the provision of in-services, CEU opportunities, promoting national certification, models evidence-based practice
o Participates in the development and review of policies and procedures
The NURSING CASE MGR LEAD takes an active role in performance improvement activities as it relates to their area of assignment.
o Evaluates training effectiveness and responds as appropriate
o Participates in department compliance monitoring and auditing
o Works with the manager and human resources in performance management counseling and discipline of staff
High School diploma or equivalent required
Bachelor's Degree in Nursing required
Registered Nurse in State of Kansas required
Minimum two years healthcare clinical experience required
Ability to proficiently read and document in electronic medical record required
